I absolutely loved this place. The nurses were awesome. Very friendly anything and everything we needed the gave us. I was even allowed to bring my neighbors dog KoKo with me for her to see her daddy.

Signature Healthcare at North Hardin is a great place that really cares for it's residents. They have caring staff that go above and beyond for the residents. Activities are a lot of fun and the food tasted great. SHC at North Hardin is an asset to this community.

This place needs a lawsuit. Lazy and incompetent employees right up to the director. My dad came out worse than when he went in. In one week he ended up in the ER with severe dehydration, a UTI, hadn't had a bath in a week, hadn't gotten his pain meds and had a broken back.

My mom was admitted for a collapsed vertebre and this horrible place got my mother sick by not giving her, her medicine. Then when they put her on antibiotics they got her sick and it killed her. This place needs to be shut down. My mom would still be with us if they just did their job.

This place is an incompetent sh*% show!! No one takes initiative or ownership in anything! From the time we sat foot in the place been nothing but problems from medication to not setting up rehab after discharge from sending patient on transportation when family taking them to appointments to many other issues and nurses with zero empathy in the entire place! NEVER will any of my family or loved ones ever be back here!! I'm

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
