My father was diagnosed with a terminal illness with in 2 weeks of his ER visit. I called North Ridge Care Center and I was very impressed with the quick response to assist my family and I in placing my father. During his stay at North Ridge, I witnessed countless patients being taken care of with the utmost respect. My family is very pleased with the care provided by the awesome staff. Since my father's transition; we have made a donation to the facility in honor of my father. Thanks to each of you!

My brother was at Northridge Care Center for six years. They were a blessing for me and my family. Northridge staff was helpful, friendly, professional and caring people to my brother. I thank God for everything they did, because I can say and I know there were times they went over and beyond the call of duty for my brother. Thank you!!! Northridge Care Center...

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
