I give this place 1 star because of the physical therapists. If it was not for them I would give them no stars. I would not recommend this place to anyone. My mother was in the place. She was totally neglected. Every time we went to visit her it was always something. She was always wet or lying on the floor from where she has fallen. It would take them for ever to come assist her when she push the call button. They would always make excuses. They are always short staffed. This says a lot about this place. One time I came to visit she was lying on a mattess with no cover cover in her own urine. I just about came unglued. I spoke to they Director of nursing and her boss. Still nothing happened. This is the worse facility ever. Before my mother went to this place she was in better shape than when she left. They were supposed to help her get better they did the opposite. If you really care for your loved ones DO DOT send them here.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
