The staff at Southern Hills were so caring, supportive, and professional, we couldn't have asked for better care for our Mother. From the beginning to the end they treated us like family. Our Mother loved it and was so happy there, she just knew she was home. We highly recommend this WONDERFUL place!

i am getting my mother out of there and Ms Ott needs to remove all the stuff on her facebook page noting how she is such a christian woman. she uses that for her own agenda. she is also a poor administrator.

I only did a star because I wanted to share. Undersraff, resident need care, too much talking, computer games, cellphone usage shen you have residents help bells going off like it a song being played, no one answers, I had to use my cellphone to get help for my mom cuz her help button wasn't being answered. How can people in this field of work be so COLDHEARTED? I was complainting on a daily basis, stool on toilet seat, floor, resident not getting changed, sink hanging off wall, bed not locked broken and was ridged to accommodate but unsafe, sends resident to dr appointments with booboo under fingernails from night before accident...Do anyone care at this facility? I can't image residents who don't have family, once I had to tell nurse about another resident had cough, turns out resident had pneumonia got hospitalized. that just one of many BAD experiences at Southern Hills rehabilitation nursing home 2019..

Incompetent. Lack of communication. Poor work ethic of nursing staff which Is a direct reflection of the administration or lack of. I would not trust this place to be responsible to take care of my loved one. And dont try to call the nurses station for assistance, you'll probably be left waiting for days. Unprofessional, and lazy employees.

If I could give no star I would. My mother was left in a pile of feces for an hour. She called for assistance and due to "shift change" the cna left her there. I had to call the nursing home (I live in California) to get someone to help her. I will be contacting the supervisor on Monday because apparently they don't have one on staff during the weekend!

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
