My mom is currently getting rehab. I had been trying to get in touch with her by phone. Incredibly, there are no landlines in the rooms. I had called three times and was told by the nurses this is not a good time. They cannot get my mother a phone and I futher inquired the reasons why and was rudely hung up on twice. First by the nurse and then, when I had called back to inquire as to why the nurse hung up on me, the receptionist that answers the phone hung up on me as well. This leaves me with the impression that there is something to hide or at the very least lack of respect for family members. The staff could never give me a straight answer about a good time to call my Mom. I am having trouble finding any help with administration or management to voice my complaints. If anyone can give me a idea as to how to get in touch with management this would greatly be appreciated.

We took my mom to Landmark last week - just out of the hospital - for some rehab time. You should NOT do the same... The perky young sales/admin lady seemed great, but it was downhill from there... The A/C was horribly squealing - to which she said "we will get that fixed right away..." -- never got looked at by anyone, muchless repaired during her week-long stay. The attendants did not seem to care about the patients - just another number... The room was sparse to say the least - a tiny manual-crank bed, spackle on the walls, 2 chairs that didn't remotely match, no TV, no baseboard moulding -- again, the admin said the remodeling in her room would be finished "maybe by the time we check-in, but certainly shortly after.." The room never got fixed! One day, the people bring my mom her medicines with water - another time they crush it up into her applesauce - YUCK!! They came to get her for PT one morning and she was not quite ready so they said they would be back for her later -- but, they never came back. PT is why she was there!! I will say the PT folks seemed very nice and caring - but can't say the same for the rest of the staff or the physical facility. Overall, just avoid this place. At the time, it seemed like our only choice and was apparently the "only bed in town" .. We have since (less than 1 week) moved my mom into a MUCH nicer, cleaner, more-caring facility.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
