Let me start out by saying there must be amazing staff but I have failed to find any. Staff was on the phone and residents were wandering freely. The Social Worker was rude and not very helpful. The place reeked of urine and other scents I cannot name. The place has rat traps everywhere, so we know what that means. I am still baffled this place is open.

Urine smell always present. Clothing is sent to laundry and very little comes back. If you go here, have family do laundry. Staff pleasant, but facility woefully inadequate for long term care.

sent my son to ICU and never called family. Place is a dump, I want him transferred ASAP I think they try to keep Medicaid patients to keep $$$$ but are lousy at care

Staff is pleasant and knowledgeable about my adult son's needs. He has been there for 1.5 years and is doing great. The staff really does care about him and manages his care in an excellent manner. They provide daily activities and frequent outings for him. He is always clean and well taken care of. The staff are very knowledgeable about his condition. I highly recommend Kindred Nursing and Rehabilitation to anyone!

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
