I just want to thank the staff for their professionalism and attention to my needs. They were all very knowledgeable about the services your facility offered. I was a visitor in your facility and the staff was amazing, they took time to answer all my questions and concerns. I am very confident that my loved one would be in great hands. The facility was clean and tidy. I also enjoyed the layout of the building down to the modern look of the place. There was plenty of outside area. I was very impressed with the facilities emotional and physical stimulation activities for the residents. I would recommend this facility to anyone looking for somewhere to have their loved one in great hands.

I work at a place that can really challenging and stressful at times; the behaviors the struggles and ailments our residents suffer from; what I do know is with all adversities and obstacles we may face on a day to day basis, I work with a great team of people that makes it all worth it because we come together strong and finish on top always with commitment and determination and whole lot of love for our residents. if I had to place my love one it would definitely be at Advantage Northwest because I know first hand how much they care.. Great Job Northwest You Guys Rock !!!!!!

Wonderful place for employees and residents. I've been employed here for 9 yrs and I've had friends and family receive skilled care here. I've seen residents come on a stretcher and leave the facility ambulating. The success of a resident comes from great care from staff as well as a supportive family.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
