oShort staffed oUnprofessional oOverwork staff (nursing, CNAs, respiratory) without any bonuses oUpper management is very rude orespiratory manager has a very bad attitude and does not know how to handle her responsibilities, so she pass her duties on to another therapist who is not qualified oDON is an airhead oThe scheduler is an idiot oThe new weekend supervisor "D" which stands for "dummy" is ghost...no where to be found or she's either sleeping on the med cart oEveryone is just tired and very frustrated oThe workload is 1 to 20...which is very dangerous. oThe doctors are no where to be found oThe residents are frustrated because they haven't even spoke or even know their PCP oCare conferences consist of floor nurses and not the doctors oI wouldn't put my worst enemy in this facility oOoooh and let's not forget their slow about giving raises oThey harass people oThe only plus is I get a check every two weeks

I can't believe that corporate hasn't stepped in and gotten rid of the Administrator Monica Morris.....DON Doreen Crenshaw........Unit Manager on 4 MFON ESSIEN and several other people including staffing, the customer service at this place is terrible, residents on 4 south are getting bed sores as well as UTI's back to back, they cant keep staff....always short due to the lack of professionalism from the upper management....all Monica does is eat and take vacation days....Doreen hounds her good workers but let things slide to the ones who play on the clock half do their job etc only because they work doubles and are men, so if you are a female coming to work here prepare to deal with work place discrimination, The state has recently cited them....the nurses have no respect for the aides, when I visit my loved one there I can only imagine what these poor cnas are going through on that vent unit, they under pay everyone who works on that unit....it is almost always 20 to 2 ratio of total care vent residents on a daily, I have had about enough of this place....currently seeking a place to move my loved one...if anybody else is having these issues where I checked bandages for their wounds and some have been on for 3 days, or not dated at all, nurses can't seem to answer questions, but sit behind the nurses station and eat or talk on the phone while call lights are going off....The RT's are just as bad....I even heard their director say " I am not about to do no shower" her name is Sabrina by the way....I pray to god everyday for me to find a better place for my loved one, DO NOT PLACE YOUR LOVED ONE HERE........RUN............EMAIL CORPORATE...........SHARE REVIEWS ON SOCIAL MEDIA............CALL FOX2 to investigate........................A CHANGE NEEDS TO HAPPEN..........IT COULD BE YOU IN THERE........THEY ARE KILLING PEOPLE!!!!!!!!!!!!!!!!!!!!!!!!!

Very bad environment due to management they lose staff and have a very high turn over rate. They dont issue the sign on bonus no let me correct that they gave all the new staff $500 cash instead of the $1500 that was supposed to be issued In the first 30 days. Strangely they issued all the nurses the amount in cash of only 500 dollars. The place is bad due to DOREEN CRENSHAW the DON and MONICA the administrator. Please be very careful about placing your love one here or coming as staff because they want get the proper care due to the fact the stressing staff out and dont have staff. Nurses are working who barely can function cause they mandating them to work from midnights to days without the adequate amount of sleep. On another note the owner's must not care because they haven't address the issue of why this company on Jefferson has a high turn over rate. Just be careful because it's dangerous and been a lot of deaths and issues since them two came in the building. Makes you wonder why would they hire a Don and administrator who was walked out from a previous company due to the DON fighting with a cna and a administrator who allow it to go on. Plus cause lawsuit after lawsuit on every company they have been at. But please explain how they issued only $500 dollars cash of which was suppose to be $1500 the first thirty days sounds like someone has there hand in the cookie jar and pocketing the rest.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
