Do NOT let your family or friends stay here! My mom was a resident for ONE night and the amount of neglect that occurred in the entire 18 hour period she was there is disturbing. For anyone interesting in staying at this establishment you better not be in need on ANY assistance what so ever. Especially to anybody that is considered handicapped at the time of arrival. If you're handicapped in any way be prepared to do everything on your own until Physical Therapy meets with you for a consult because the nursing assistants make it very clear you will get NO equipment to assist you, you can NOT bring any equipment in for temporary assistance or they will take it from you, let alone help assist you themselves. (Equipment as in a walker to use in order to be mobile enough to get in and out of bed, wheelchair, etc.). The only assistance the night shift nursing assistants can help with is placing patients in bed and then making it very clear they are staying in bed until Physical Therapy meets with them, regardless of any circumstances that may arise! Since that was the case then the staff must obviously be able to accommodate to the patients that are in need due to them not having the proper equipment to be mobile themselves but that unfortunately was not the case. Also, be prepared from them to not listen to instructions or doctor's orders, such as to be Non-weight bearing on one foot. I pulled her out and made other arrangements elsewhere. Obviously she was not the only patient there and others needed assistance as well, however when the night shift is ignoring everyone and not answering to help calls or when they finally do have an attitude problem and roll their eyes and scuff at patients that's an issue! Obviously the night shift gets upset when they are disturbed from their busy schedule hanging out in the lobby watching TV and enjoying themselves hooting and hollering. Do yourself a favor and save the headache and DO NOT bother with this place!

Great place. Wonderful staff. The admission nurse Robyn was wonderful. Always checking on us to ensure we were taken care of. They were wonderful with my Grandmother.

I had a friend in Wellbridge after spine surgery. The place is very well kept, good food. But most importantly, the staff is the best! Great place for rehabilitation therapy.

They treated my 92 Mom ( Phyllis M ) like a princess. Excellent care. Great food. Always on top of things. Thank you Robyn in admissions for all your help. I just wanted everyone to know that after reading many reviews and visiting many places that I picked a winner.

The neglect at this facility should be investigated for Elder Abuse. I wish more families took time to write reviews. My mother was at this facility for 20 days and the care has been heartbreakingly awful. Incidents where my mother (Above Knee Amputee) requested help for the bathroom and was told to go in her depends. The response from Sr. Nursing staff was that they had this complaint before regarding this nurse & would address it with her again. The Physical Therapy was at most 45 min a day. My mother laid in bed while her children read different ways to get her help/resources for when she gets home. Her true rehabilitation will have to happen at home. There are many choices. Do your homework and believe those families that have taken time to write reviews (and not hide behind bogus user names with 5 stars) that this is not a place you would leave a pet at let alone a beloved family member. I can't warn you enough to stay away.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
