I have worked closely with this building as a professional and from a personal experience. Both of my grandma's and aunt have stayed here on their TCU unit & on memory care. The staff are friendly and were attentive to my families needs. I would highly recommend Birchwood Care Center!

Very caring staff. They are very good to my mother who is on Hospice Care there. Kudos to all who do such a nice job caring for her!

Please don't take your loved ones here. The care is a disgrace to the nursing profession.

Very unpatriotic. Will not take CHAMP VA for a veteran's dying spouse. They know they can, but are too lazy to.

The worst experience at this nursery. I was a intern here in the past. I don't like how the HUC supervisor were rolling her eyes at a patient with Alzheimer. I didn't get much training done and the assistant pressure me into signing nurses date forms that I wasn't authorize to sign. This place shouldn't ever be held for intern people to be treated this way and lied about. I pass my class at the top of the class and some of what they taught me here had terrible medical terminology. I was also being talked behind my back instead of being talked to about my mistakes. This is suppose to be a learning process and I didnt get two days to prove myself before I was booted out. Thanks alot burchwood. I have talked to the program lead and hope things change for the better already. The young female lady that i shadow should be fire if she is able to pressure me to sign nurse forms that I shouldnt sign how is she trusted there with patients.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
