DO NOT BRING YOUR LOVED ONES HERE. THEY NEGLECT ALL THE PATIENTS. IS SO SAD.

Staff here was always so nice and friendly. Place was always clean and well kept every time I came to visit grandma. Highly recommend this place for your loved one.

My mother broke her hip so we needed a place for her to recover. This place is located in a quiet, residential neighborhood in Osseo. The Villa is a section of the nursing home reserved for physical rehab that is new and very nice. Her private room looked like a hotel room with her own bathroom and TV. The physical therapists are excellent! Family members can eat with residents for only $2.50 a meal, and it is so delicious! I had roast with potatoes - which was better than anything I could make. Mom was sad to go because they had taken such good care of her. We all would highly recommend The Villa for anyone needing physical rehab.

Renovating a building doesn't equate a quality facility. Often understaffed, high turnover and a lot of bad attitudes. Former employee here, witnessed prejudice from administration, mediocre to subpar care, regional team could care less and is also petty, they spread staff thin like you wouldn't believe. One of the highest turnovers for a reason and it's not just this location, you are replaceable, and these places don't change until management does, villa shouldnt be it.

Fines, Complaints, and Inspection Problems in the Past 3 Years

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
The Villa At Osseo has not received any fines, complaints, or inspection problems in the last 3 years.

Staffing Levels Per Resident per Day
