If I had to choose one word to describe my experience with Oak Forest it would be compassion. All staff members are always patient and understanding in difficult times. There is always some one there to lend a hand and offer assistance. I would recommend Oak Forest to any friend or family member seeking nursing home placement for a loved one.

This place actually gets away with murder. They were extremely negligent, especially since the patient couldn't speak up for herself. The wound care was a joke and the nurses were clueless. She left that facility with a big gaping hole. They just didn't care. My loved one was constantly dehydrated and malnourished. They wouldn't even take time from their busy day to give her WATER nor would they feed her. It really needs to be investigated. Do not use this facility unless you're trying to get rid of someone.

This place is as good as you are. If you're absent, the staff is absent. The lack of patient knowledge is shocking. If the rest of the staff was as good as recruitment I would give it five stars! The Administrators are awesome at getting you in with a table full of folks telling you their plans for your love one. What they should have told me was, "we're going to keep your family member pissy for you and make sure you can see all the secretions on the gown so don't be alarmed. We will make your family member presentable for doctor's appointment and hospital visits though. We check out on the weekends so do not bother to ask us anything". Last but not least, everything you complain about you can count on this response, "Oh, I, they, or we just did it"! I can say more but this is enough!!! The pictures are heartbreaking as well. Thank GOD for the roommate, at least they're seeing about that resident and can check on my family member from time to time. SOOOOO if you want constant headaches, emotional distress, high blood pressure, and a slight drinking problem. THIS IS THE PLACE TO BE! Good luck! God help us ALL!

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
Oak Forest Health And Rehabilitation has not received any fines, complaints, or inspection problems in the last 3 years.
