We were very lucky to have my mother move from another facility to this one before she passed. At the prior facility she had fallen several times and one time was left on the floor for hours. No one checked on her. After another fall where I had to transport my mom to the ER, we never returned and we moved her immediately to Nye Pointe. The staff there was wonderful. She was well-cared for. When she was in the process of dying, the staff along with her hospice care-givers could not have been more helpful. The lesson I learned is to look at the staff. If they treat their residents as humans, they will do their very best to make their lives comfortable. Both my husband and I volunteer there now. It's a great facility with staff that really care.

I wouldn't even think about placing a loved one here. My grandmother was there for after stroke care while we were there the nurses weren't nice and when my grandmother needed to use the bathroom it would take them FOREVER to get to her in CNA school I was taught 5 mins or less to answer a call light and they took a long time to answer my grandmother had kidney infections and everything one day it took them 3 hours to to take her to the bath room!!!! Things also came up missing from her room and it was a bunch of different things. My grandmother is now passed away and its really sad to think this place helped push her to death her kidneys failed fast and it took her. The state of Nebraska needs to shut this place down!

I'm writing this review/complaint in regards to my mother's treatment at Nye Pointe. She couldnt use the bathroom by herself, and she'd be left sitting for and waiting for someone to take her so she could releave herself. instead she was left for several minutes just sitting there. My Mom had to call a family member to come help her go potty. When this family member walked in Nye Pointe she was surrounded by so many employees there, but not one of them took the time to give my mother a helping hand. My mom had an appt with her surgeon in Omaha. The morning of her appt.(last week) she was sent by van, with no breakfast, also her scheduled pain medication was not given. We later asked about this, Nye response was SHE needs to ask for it. Ridiculous. I can only imagine the many other things that could of been happening to my mom there at Nye Point. This all took place while this facility was under a State review...mind blowing. We had her removed that day from this Nye Pointe rehab. She's now in safe hands and she's happy.

Facility transportation on Wednesday was 1/2 hour late getting to appointment. Patient had a doctor's appointment Thursday and was transported by the facility to that appointment she arrived one hour early but she was not given her medications or breakfast before she was taken to the Dr appointment.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
