Recently I had an outraging experience at Southgate. I brought a member of my family a cinnamon bun, and a fork was requested with which to eat it. I did not have one with me, so I went to ask the nurses at the desk if they had any plastic forks and knives I could provide to my family member. They *laughed* at the question, as if the notion that the patients under their care were physically or mentally capable of using a fork or a knife was inherently absurd to them, with much the same tone of voice and demeanor that one would expect a grade-school librarian to adopt when asked if they had any books on quantum mechanics to provide for their students. This comes on the heels of an incident in which they let said member of my family go without breakfast, being unwilling to wake them for breakfast, and then having removed the uneaten breakfast before the family member awoke.

There are plenty of nursing homes out there and some are good and some are bad but South Gate is horrible from my family and I Experience NEVER send your love one they have some good CNA but for the most part the cna that work in South Gate are horrible .. they had my grandmother in bed for lord knows how long soaking wet in pee I have no idea why they tried it because my family comes to see my grandmother every and then they had 2 cna who was supposed to be in there changing my grandmother and they playing in there phone that's not the worst part ... these people left my grandmother with a fever of 102 and had her staying at the nursing until the next morning instead of calling 911 that night they waited until they next morning and only called transport she had a fever and she was in respiratory distress when I walked in that morning my grandmother air conditioner was off and it's supposed to be on cool at all times when it got turn on the settings was on heat my grandmother was so hot it felt like she was laying in the sun all day SOUTH GATE ALMOST KILLED MY GRANDMOTHER if you have family that you love and care about DO NOT I REPEAT DONT NOT SEND THEM TO SOUTH GATE unless you want them to DIE ?????

Rehab was very good, got me back on my feet and my strength came back , they worked with me and I am grateful for the whole staff at South Gate, Great care there

Staff sits on their phones and chat amongst theirselves. Get three meals a day but no one comes around asking about drinks, snacks or if they need to use the bathroom. Poorly maintained and dirty. Stay away from here.

Amazing Experience! The staff cared for me like I was their parent. The place is really nice and clean. Everyone is so warm, caring & friendly. The care is absolutely amazing!!!

Fines, Complaints, and Inspection Problems in the Past 3 Years

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
