My grandma was punched in the face and we weren't I formed. There was a lot of back and forth to get answers. I strongly believed it was staff because any other given time we would've received immediate notice. No ice on a huge knot she had on her forehead. The beds dont have bedrails to prevent falls. Every time I am there, i don't see them go to her room to check in on her. A lot of the times she's on her own and who knows for how long before they found her on the floor. I've called countless times and resident pickup thenurses station and dayroom phone and then they hang up. I don't live in the state and I am not next of kin, but I don't like the nursing home or her condition there. They leave their residents to die there.

Whatever you do don't allow a loved one to go to this nursing home. It is the most depressing place you will ever be in. There is very little activity for the patients. Most of the staff is very unfriendly. I only met one staff that was friendly. They truly don't care about you. They really don't help you. When you decide you have to get out of there it's good-bye Charlie. If you have nowhere to go it's not there problem. They will not help you or refer you to a safe place. It's a sad place.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
