My mother was placed in this facility during the final weeks of her life. The staff was amazing to her and our family. They showed genuine care and concern for my mother, comforting her during her final days. We were always greeted with a smile and warmth from the staff members. It put us at ease knowing she was being well cared for during this difficult time.

Nothing is worse than having a loved one that needs help you just aren't able to give yourself. My mother broke her femur and had major reconstructive surgery. This place has been terrible. They do not follow doctor's orders, they misdiagnose, they don't get meds out on time. They treat their staff horribly to the point that they are ALWAYS understaffed because of people quitting. If there is ANYWHERE else for your loved one to go, send them elsewhere! **Update** This place discharged my mother with undiagnosed blood clots that eventually caused her death in January of 2017. Do not DO NOT use this facility.

I feel really bad for the patients here..... The staff is lazy, have been found sleeping in the empty rooms, and don't care for the patients that they have...... Would never suggest that you bring any elderly, disabled family members or friends here to be taken care of

Dirty, smelly, and the food is disgusting. Don't subject your love one to this horrible place. It is not safe! The day I took my husband out of there, 53 New York State Health violations were displayed by law on the receptionist desk in the lobby . How many violations does it take to close this terrible facility with a negligent unprofessional staff.?? Beware!

this was and is by far the most DISGUSTING nursing home I have ever seen. the nurses are ghetto and unprofessional, they literally look at you like you have three heads when you say hello or act professional. The owner of the nursing home is off spending all of the money he is receiving for care of patients on escorts. it's a joke. I worked here for 5 days and I was heartbroken every time I went home by the gross lack of care I saw given towards the patients. They will give other patients medications that don't even belong to them as if it's ok to do that, when I would come in at 3 p.m. the patients would still have food on their faces from lunch time. DO NOT EVER put your loved one in this dump.

Fines, Complaints, and Inspection Problems in the Past 3 Years

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
