Our family has had extremely positive experiences at the Osborn in both the Pavilion (skilled nursing area) and the Assisted Living apartments - where our parents have lived for several years. We believe that the Osborn's care is absolutely top-notch; it is delivered with kindness and professionalism. The staff has been wonderful in all areas, particularly the Osborn Home Care aides. When our questions or concerns have occasionally arisen, they have been promptly addressed. The Osborn's facilities are upbeat, elegant and well-maintained. The grounds are absolutely beautiful to look at year round; there's nice seating for the warmer weather. There are a number of good dining options for residents and guests, which we enjoy when we visit. Before our parents came to the Osborn, we didn't know that such a wonderful place existed. We're grateful that it does!

I was so lucky to have Osborn Home Care for six weeks after my surgery. The kindness and quality of medical care was better than I ever expected. My aide was cheerful and always brightened my day.

First impression is very important. Came in this morning. Went to the front desk to ask for a directions to get to a room. I said "good morning" and the lady at the desk didn't even look at me. No good morning, no nothing. I guess she's not "front desk" so it's not her job greeting people. It's my first day at this facility. Once I get her name I will come back to the review and say who she is.

The Osborn provided loving care to my profoundly disabled mother-in-law for 16 years; the staff became like an extended family to her, and to us, helping us keep her spirits up while they tended to her many needs. Together we also enjoyed their beautiful grounds during our regular visits, it felt like a gracious home, full of elegant, traditional furnishings and bright, cheerful colors. Everyone who came with us to see her thought it was the best place of its kind that they had ever seen, and we certainly agreed.

Fines, Complaints, and Inspection Problems in the Past 3 Years

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
The Osborn has not received any fines, complaints, or inspection problems in the last 3 years.

Staffing Levels Per Resident per Day

Medicare determines the expected staffing time per resident per day depending on level of care the residents of The Osborn require. It is important to compare the reported time to expected time for a single facility instead of comparing the amount of time per resident of two facilities. Learn why.
