Horrible place! The nurse was laughing because my mother fell and there was blood everywhere. After I had spent 30 minutes on hold and was told there is no one at the desk that they just answer the phone if they walk by and hear it by chance.

Short staffed, sometimes only one person at a time for a whole wide area. People not supposed to be walking were walking down hall, man in wheelchair slid out of it and fell to the floor. Visitors trying to get in couldn't until someone set off alarm opening it for them, which continued to sound off for maybe ten minutes, while several patients unattended. My mother was a high fall risk and fell several times, and received a gash on her tongue one of those times after hitting her head on the wheelchair. Some rude or unprofessional staff. Fairly unclean. Lost her glasses and would not help me locate them. Discharged her day after care conference in which nothing of discharge was said. Place is a nightmare.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
