My Mother lived there for nearly nine months following a fall and subsequent four surgeries. The exceptional nursing care and physical therapy she received there were the only reasons my 85 yr old Mother was able to rehabilitate and eventually move into my home and live with my family! Despite several devastating setbacks that appeared to make us think she would live the rest of her life in a facility, she regained her ability to walk again and become incredibly independent once again. I cannot say enough about this amazing staff for not just being caregivers but becoming true friends. They remained vigilant in her care and never gave up hope. When I left after visiting, I knew she was in the best possible hands. It was a bittersweet day to say goodbye to our friends at The Avenue after nearly a year. I can't think of a better tribute to a facility than that!! Highly highly recommended!

A beautiful facility. Staff that "appeared" to be very nice and welcoming when they were "selling" their facility to us. After less than a year they decided to kick my mother out on the streets, basically, because she has Dementia and has problems with anxiety. They find her to be annoying. She has never been violent. She has never tried to escape. She is able to independently care for herself physically. She is just anxious. They can't handle it. They basically seem to want a nursing home full of neat, clean, happy, easy to care for elderly who are not difficult in any way. They advertise themselves as a "skilled" nursing home. That is false. They could not handle simple anxiety in an elderly person. They waited to evict her, though, until after all of her Medicare funds were gone. Now my poor mother will have to try to adjust to a new facility. It will be terrifying for her. She would have been better off starting and staying somewhere rather than being moved twice in a year. Beware of this facility.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
