My Mom was here for rehab, when I refused to admit her for very legitimate reasons the social worker their called Adult Protective Services. Now I have to deal with them. They made medication changes for my Mom took her off most her meds very dangerous. Allowed a newspaper to take my Moms picture and publish it without my permission or even informing me I found out by someone showing it to me and asking isn't this your Mom? HIPPA is not something they understand at all trust me and they allowed a family member to order my Mom to the doctor and then wanted me to pay the bill. Avoid this Place like the Plague....

This place sucks they are the worst nursing home to send a loved one to. they are the lowest paid nursing home in all of ohio. they have teenagers working there for minimum wage. i dont know about you but i dont want some teeny bopper taking care of my loved one. i would red flag them if i was the health department they have roaches and mice running in and out of the place and recently a lady died from a spider bit.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
