Do not send any loved one to this facility. The administrator likes to belittle women. He also tells the young female staff that he is their DADDY. Even after several complaints he is still working there today. State is in this facility all the time due to being under staffed and lack of care. Please stay as far away from this facility as possible. There are good Grace facility out there, but this is not one of them.

Good place to live. Needs more Aide staff. But they treat me very kindly and with loving care.

Extremely impressed with how clean, educated, and proficient the staff was. Each one carried a smile that was impossible to falsify. Each time we visited Dad, there were 2 or more staff members in the Administrators office ready to absorb information as he stayed ready to teach and build them up. It was a pleasure coming to visit and watch this health service organization grow into what it has become over the past few months. -Brittney Brown, Ph.D., SPHR, SHRM-SCP Professor of Management at Northeastern State University

My mom's friend sat in her own poop for 2 days, and has had ecoli multiple times since she has been at glenwood. Getting them to bath her or change her sheets is like pulling teeth. They only give her half her meds. I wouldn't send a loved one here unless you want them to die soon.

Very poorly ran facility. Glenwood is always understaffed due to the mistreatment of their employees which causes poor patient care. State is there on a weekly basis which says a lot. I would not recommend sending a loved one to this place at all, especially if they are unable to speak for themselves.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
