If I could have given zero stars I would have. This place is a joke. I heard " I don't know" more than anything else from the staff. The food was horrific and portions were so small. My dad was constantly hungry and asking for me to bring in food. It was lliterally the bling leading the blind. The only nice experience was from the therapy staff whom didnt work for them directly. The nurse thought my dad was there for a stroke, my father did in fact have a stroke only 40 years prior. They knew nothung if gis histrory nor did they attempt to learn. Terrible place...

They let my Uncle die alone instead of notifying family that he was transferred to the hospital. They let a bitter woman isolate a man from his concerned family. They let her emotionally cut him off and isolate him. The definition of abuse. And did nothing.

Mother in law was transferred here after having back surgery. They made her sign papers under opiate pain killers. She didn't know what she was signing, happened to be a No Treat form; meaning she gave up all rights to treat her that includes a stomach pain. She was complaining about her stomach since she got there, we ended up going to the facility one night at 9pm. The caregiver (no schooling behind her) told us that earlier it was ONLY nausea. We demanded the nurse to check her out, since we recognized she was in alot of pain. The nurse told us she wasnt aware that she was in pain no one told her and she would send someone in to take her vitals. Well the person that was sent in to take vitals didn't even know how to put the blood pressure cuff on right, they came back and said they called the doctor and the doctor said that her vitals wasn't bad enough to call an ambulance but if we wanted to take her to the ER by personal vehicle we could. We did take her, and the ER ended up admitting her to the hospital. If there team at this facility communicated and worked as a team my mother in law wouldn't have been admitted. We haven't talked to the facility since we left there, it's now 10:30am the next day, and they haven't even called to see how she was doing or where she is at. Not a place I want her to go back to for sure.

Fines, Complaints, and Inspection Problems in the Past 3 Years

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
