I was very impressed with the staff and programs offered a Fox Subacute. I felt comfortable as I was given as much time as I needed to ask questions to understand how the process and programs work.

My interactions with hospitality and medical staff has and always been stellar! The level of detail and care they have and give patients and family are outstanding.

I was a patient at this facility and am now on my next steps to recovery. The food here is not good at all, you better hope you dont have a life threatening emergency as they keep it so understaffed a lot of times that it takes a while for you to get help there is only one nurse and 2 assistants per 20 patients at some points in time and there really anything to do. and the reason im changing my 1 star to a 3 star rating is that the Therapy department OT and PT seem to have a very genuine care and interests in the patients needs this department was by far my favorite group of people in my time at Fox

Very understaffed. Caregivers tend to be vindictive to patients.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
