I was very surprised by this facility. I highly recommend anyone looking for nursing or rehab care to look into Concordia. They seem to be a very well structured company with the patient and their well being, being the main priority. Amazing rehab, great nursing staff and caring CNA's. For being a nursing facility I was surprised on how well staffed they were. Great place for loved ones to be cared for.

Utterly awful place. Severely understaff. Staff is rude and uncaring to the residents. Nursing staff not much better. My mom was there for 2 weeks without a single shower. I could go on and on.

It's obvious that the last 2 good reviews were from people who work at this facility, considering Nathan beers left a 5 Star review 10 months ago and now he's leaving another detailed review. Maybe if you were worried about your patient care instead of leaving fake reviews on your google page your patients would be properly cared for and residents and their families wouldn't have to leave negative reviews in the first place. This facility obviously has no integrity and I definitely wouldn't trust my loved ones with them. I hope this place is being investigated by the Department of Health and Human Services, because due to the reviews left by REAL patients and their families is looks like the staff is at the very least negligent with their patients.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
