I visited Greystone for the first time today and was so impressed with the gorgeous grounds and beautiful atmosphere! The staff are so devoted and caring, they work as a team to provide top notch care 24/7 for patients with complex medical needs. I would definitely recommend Greystone for both short term rehab after an illness, injury or surgery, or for long term care. It is a warm and home like environment with skilled medical professionals on staff. Love it!

My mother was transferred to Greystone after being at another facility for 5 weeks and no improvement. In 3 weeks my mother is already getting awesome care and therapy. Her vent weaning is advancing at a steady pace. Staff talks to her and their caring attitudes have given us and my mom hope again. Thankyou all at Greystone!

I have worked here at Greystone Healthcare for the past four years and I would not want to work anywhere else! We here at Greystone are not only employees we are a family and by working together we have made Greystone an awesome facility!!. We have worked so incredibly hard to ensure the care provided to each resident is up to our expectations if our loved one was in a facility. Since the addition of our new Administrator and DON, the atmosphere in the building is so very different, you can walk in the building and it feels as if you are coming home. We have an Awesome IDT team , Incredibly dedicated and phenomenal Nursing Staff, Respiratory Therapists and CNAs who actually care about the job they do everyday! We have a great Therapy department who not only are able to get the patients motivated to start therapy they are continuously encouraging them to do and be all they can be!. Please if you are considering placing your loved one in a facility, come and tour our building, take time to meet all of us and see what a truly great facility we are!

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
