Not a good experience, take your loved one elsewhere, was messed up from day1,We was there 2-3 months, pills left on table not taken for several hours (they are suppose to make sure she takes them)we left @3.30 on a Friday afternoon & they didn't even give her 5pm medicine that left us scrambling to get some,we called Dr.Paul Yim,hoping to get some samples or a prescription to get us through the weekend,he refuse unless we brought her in ( we had just checked out) no one seems to be connected there, the nurses were mostly nice but very slow to respond, once I pushed the button for her to to bathroom & sit there for 30 min. With a guy sitting 40 ft away, just a lot of Lil things that wasn't done properly, mri shows my mom had a stroke within the last 7 days & she's been home for 3 days, several days before discharge she was acting funny, did they forget her meds?? Hope this helps someone else looking at nursing home

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
