I love working at Brady West. Our residents have become my extended family. It is very rewarding to see them become more independent and to see their progress during therapy. It makes me proud to be a nurse at Brady West when I can brighten my residents' day, put a smile on their faces, or even be their comfort while they go through the end of life process. My co-workers are great at helping me any time I need it and I love that. Our facility has come a long way and has made a lot of changes for the better.

Brady West Rehab & Nurse Center is not "just a nursing home." We are a living breathing family. Residents are treated with respect and dignity and plenty of TLC. We pride ourselves on having many activities for our residents, including country music bands, parties of all sorts, lively 42 games, bingo and so much more. It is a goal to promote each resident to be all they can be while living here. Our resident's families as well as the resident know we have an open door policy to all of our administrative personnel. I have been an RN for 46 years, many of those involving geriatric care. I will gladly recommend Brady West Rehab & Nursing for your rehab. Come and visit us and see what being a "family" is like.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
