Run Away Fast.. DO NOT let your loved one stay here. This place is a disgrace. My mother stayed here for a short period of time due to a fall and we were so happy when she left this place. Old and out- dated furniture, patients gowns and socks are so worn out that they should be trashed. The bedsheets have holes in them, the pillows are ripped and torn, the pillow cases are faded and appear to be unclean. RESPONSE WAIT TIME - I personally timed how long it takes a staff member to come and help my mom and it's anywhere from 12 - 20 minutes which is entirely too long which created other issues. Also, another patient wondered into my mother's room twice and we had to go and get a staff member to help remove them from her room. No one even noticed this person was missing. So sad. FOOD - By the time she received her food it was always cold and dry. Most of her food was bought in by family members. There are two public water fountains- one was backed up with water and had mold in it, while the other one and bugs or small ants crawling all over it. While some of the staff were nice this place was and is still a disgrace. I will never recommend it to any of my friends or family.

This place do not even deserve a star, actually this place should be SHUT DOWN for OVER MEDICATING the residents, My mother was there from 2018 of May to March of 2019, didn't even make it a year, the staff is rude especially the so call nurses they are rude Jessica that works the day shift that wears the skirts every day that nurse should be FIRED for unprofessionalism, unconcerned no compassion human being, if it wasn't for my own Nursing licensing to protect I probably would have put my hands on that woman, I had to pulled my mother from there had her transferred to Baylor Irving to get her system flushed out come to find out due to a toxicology report done they had over medicated her on psych meds to keep her sedated she had stopped eating, started falling everyday, stopped talking, and stopped walking and could barely move if it wasn't for me being a nurse and saw the changes in mother she would have probably been DEAD today, Baylor Irving Doctors said if I had left her there she would have probably been dead today, this is a disgrace of a place, they not use to family members being involved with their family members care they assume that my brother and I would be no show children but they were mistakenly wrong, I did my mother showers just so I could monitor her skin and make sure she wasn't being abused in any kinda of way, I even did my mother laundry cause i didn't want no other residents walking around with my mothers clothes on, I honestly think the state should be involved in this facility, we do have lawyers in place cause they messed with the WRONG mother, the only reason my mother was in Lennwood cause of her Dementia she would wander off from home so we had to place her in a home, but now her Dementia has advanced all because of Lennwood Over Medicating her my family will not rest till this place is shut down so anybody thinking of putting your love ones in this facility please DONT if u want them to LIVE I couldn't even post this without clicking a star now this is some fraud

My best friend's mom is here, supposedly recovering from a stroke. Anything BUT recovery and reahabitation is going on here. Nurses do not read and/or follow chart directives at all. She does not receive her showers, physical therapy or nutrition as scheduled. Two different times we popped in for a visit this week, her mom was lying in her own filth, in a fetal position, with her face smashed against the bed rail. Absolutely DISGRACEFUL!!! These are PEOPLE! Someone's parent/grand parent! We had a meeting with a nurse, the p.t. mgr and head social worker soon thereafter. Excuses galore. Folks, REALLY DO YOUR HOMEWORK before you consider placing your loved one here. We are working on getting her transferred out, as she is painfully, evidentially declining in health and spirit. Shameful....very shameful to treat anyone like this. I have cried several times after leaving.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
