I can assure you that although our facility is outdated, we have some of the best members of an interdisciplinary team that any nursing facility can ask for. Do mistakes happen? ABSOLUTELY, in every place however we truly do our best to address each concern to the best of our ability and control. We are currently under renovation & invite all individuals to review our surveys from the Department of Aging and Disability as well as invite you to come in for a tour.

My Grandfather was admitted to a SNF on the Westside. As the days went by, my grandfather fell three times in less than a week. Had bruises, skin tears, and developed some wounds. He was first admitted for Antibiotics, but ended up with more problems. We as family decided to move him to Mountain View after a week in a half. And that was the best decision we made. He got better, started walking, and wounds healed. My Grandfather made friends with staff members and residents that he decided to be there Long Term. He then spent 10+ years there at Mountain View. I would recommend Mountain View to anyone. Great Place, Great Atmosphere.

If I could give this place 0 stars I would! My grama has only been there a little over a week and already has had 2 accidents... The way I see it, is if you're not capable of taking care of other people in need than you're in the wrong profession... Every single patient in that facility needs care and I know that it must be hard to attend to each and everyone of them, but just because some of these people can't walk, talk or even move doesn't mean you treat them like they don't matter... Now I'm very upset that my grama has to be there... I wish she didn't have to be, but we have no choice... I pray that someone at that facility sees what I have wrote and at least talks to the staff to be more careful with their patients!

My brother in law in this facility and he is not being treated with the proper care there. The place is very outdated, understaffed, they want to be all in your business. This place is only out to get peoples money. And they put you on the wrong floor when your only there for rehab they put you on the permanent residence floor.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
