All I can say is WONDERFUL PLACE AND THE GREATEST STAFF. After multiple surgeries at a hospital, I came to this facility in a stretcher unable to walk and left walking out the door 2 months later. This is do to them having the best rehab facility and staff on site. -Leslie Roman

This building may not be the newest on the block, but definitely the cleanest! The staff have truly created a family environment, and the skill set they possess is over the top!

It's the best home away from home! I am able to create my own room to fit my needs and make me feel at home. We have daily activities which include, crafts, puzzles that we work as a group, we have entertainers of all genre all through the month including our monthly Polka night with food, friends and family. I highly recommend Legend Oaks Ennis!

To begin let me say God is good. I have had to believe in God to hear and answer my prayers a lot. I would like to share with you how he answered my prayer last August 2018. I was not happy at all where I was at. I will give respect to the place where I was staying by not giving there name,I wish I could give their name to protect others for moving in that place. I knew I needed to get away from that place as soon as possible I cried out to God to open a door of another place I was in the hospital and the doctors and nurses did not want me to go back to the other place and 3 doctors and 2 nurses suggested that I try to get in this place. I ask God to open the door at this place if he felt I would be better off. Well Praise God He opened the door for me to be here. I ask God to open the door or shut it and not let the devil interfere in any way. Yes I know for a fact that God opened the door because when I came in the front door on the stretcher I felt the presence of God so strong I thought I was floating in the place where I was to be. Let me say the staff is very professional The staff shows true care and concern for each person. I feel they are God sent. I know I have wrote a lot but what I have wrote is from God. So please if you need a place for a loved one you need to ask God to speak to you about this place. I have no family members that can come visit me but maybe once a year but want to feel like family this is where to be. Thank you for taking time to read such a long text.PLEASE PLEASE PLEASE show your loved one you love them and put them here so you don't worry about them being taken well care of because I have been here going on a year and I can still say THANK GOD FOR THIS NURSING HOME AND FOR SURE THE STAFF. I am willing to talk to anyone one on one so just contact me. I wish I could thank every employee here by name but scared I would leave one out. Hey, SKYLAR

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
