Bender Terrace just had our Annual State Survey January 31 to February 2, 2017 (3) days and the State surveyors left with four deficiencies and two days after the State called and they are dropping one of them. So now we only have 3 and one of it is a waiver. This is unheard off. Kudos to the loving and caring Bender Terrace staff for a job well done. We had our Employee Appreciation Friday, February 10th and many people from the community were invited and they all came. Please come and tour our facility and see the difference.

My mom was placed there recently for rehab after hurting her shoulder. It was a horrible experience for my mom and myself. She was in their for 5 days before they even bathed her. Most of the staff was rude and made ugly comments. I went in one evening and I seen a staff member running around in her socks. The building my mom was in smelt horrible. The food was bad. Hot dogs for dinner every night. Most nursing home gives you a menu to choose from. It looked old and outdated. The hospital room at university was better than these rooms. Please if you can avoid it dont put your loved one here.......

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
