My father is at Prestonwood Rehab and we have found the staff to be the most caring, kind, and hard working group of individuals. From the nurses, physical therapists, the aids, to the lady that cleans his room are better than other facilities he has been in the past. We checked out several in the Carrollton area. Dano did an amazing job and is thoughtful. All of the staff try hard to assist my dad in a professional and respectful manner even when my dad is not kind or respectful. I wish I could name all the staff by name here.

It's just a business to them...they try to upset everything....they lost parts of the specialized brace provided for my father by the hospital. I would not recommend taking your loved ones here when there at so many other options available.

The most unprofessional service I've seen in awhile , while discussing issues with no one attending to my mother in a decent and timely manner the supervisors first response was you can leave and look into other rehab facility , no apology or solution options to fix the problem ,just basically if ou don't like what we do leave, sure thing! The nurse meagean was thankfully apologetic and nice about the situation ,but 1 nurse to 21 patients is sad. And the manger proceeds to reiterates that there are 2 patient actively dieing on the floor and requiring a lot of time and attention ,I thought are y'all killing people here? Will have my mother away from this place immediately.....also the food quality and service is horrible cold, reheated processed food .

They were awful... Stay away! They could not motivate my mom at all. Takes almost 30 minutes to respond when she presses the button for someone to help her. Don't have them care for your loved one.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
