Patriot heights is constantly accommodating to my needs. My family doesn't live San Antonio so it's difficult to have them bring me in items but patriot heights goes the extra mile and helps my family out by making sure i get what I want. They have a lovely area to seat outside and watch the birds and get sunlight. I love to play bingo and bingo they have. They have all types of prizes, I normally pick jewelry and sun hats. The past 2 years I spent at patriot heights have been great. I need continuous care and they provide it even when I give them a hard time. And the housekeepers are the best always talking to me and taking the time to listen to my concerns.

I am a current resident of patriot heights. I've been living here for over four year. I come to almost every scheduled activity that is offered at patriot heights. My family and I have encountered some issues during my stay of four years but staff has addressed everyone promptly, always keeping my family and I informed. No place is perfect but patriot height is close to perfect. I am happy here and I am glad to call this my home. If you ever have the chance to come by ask for a strawberry milkshake there the best.

My Dad had an accident a little over a month ago and spent 3 days in the ER then was referred to a rehab/skilled nursing facility to help build up his strength before sending home. He is 93 this week. He spent a month at brookdale and was sent home yesterday. This place is staffed by people with real concern for the welfare of their patients. no place is perfect but I gotta say we were so glad we picked this place for his care. He is in better shape than he was before the accident. Don't want to sound like a fake review but we would recommend this place to anybody. These are the good guys doing what they do for the right reasons!

They don't treat people like humans. Edit Called the 800 number below. Tried to have a discussion with these people. They don't care, and blew me off. We have had 2 meeting with the staff concerning the sub-standard care my father is receiving. The management does not have the decency to show up to meetings they schedule. We have caught them lying several times to us. We have documented these and complained to the state of Texas. They are constantly having staffing issues. There have been 2 nurses terminated due to the treatment of my father. They were stealing his pain medicine, and not giving it to him. They let patients lay in soiled sheets and clothes for hours at a time. They don't feed patients when they are supposed to. They will set a tray of food in front of the patient and come back 1-2 hours later to feed. It should be a crime how bad they treat their patients, oh wait it is. I have filed a complaint with the state of Texas. RUN AWAY FROM THIS PLACE. DO NOT TRUST THEM WITH YOUR LOVED ONES, YOU'LL BE SORRY YOU DID.

My wife needed extensive physical therapy. On the 3rd day they saw her walking about 50 feet. They said she was okay and discharged her 2 days later. They took her by ambulance back to the hospital. The doctor said she needed more physical therapy, go figure. My wife and I pleaded our case to deaf ears. To say the least I am very angry ?.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
