My aunt, who will be 98 years old in the next few days, received rehabilitation from Carriage House for the last few weeks. She received wonderful care and excellent therapy and was able to return to her prior living arrangement sooner than expected. Thank you everyone at Carriage House. Quality care, friendly and caring relationships for both the patient and family. Thank you. Suzanne Thomas Bankston and the entire family of Merle Synder

Do not put your love one here. This place is terrible. This weekend I witnessed a man that could barely speak, finally get his words out that he wanted water, after an hour of trying to speak. The lady wouldn't help him and he had so much thickener in his water that you could have frosted a cake with it. She said she was too busy (they only have less than 12 patients in the cottages). I added more water and it was still too thick. She said "Thats ok, he is used to it." Luckily another aide came and said, no that is way too thick and fixed it so I could give it to the poor man. Then, last night, I find out that one of the aides threatened my mom and told her anything could happen to her (like she would kill her and we would never know). My sister came down today and now they are moving my mom to the old part (even though she is paying for the new side) and not suspending the aide because they feel she didn't do it. My mom didn't even want us to say anything because she was afraid they would be meaner to her. They treat their patients like crap. If they ring the call button they don't come for an hour. When I go to visit, they are sitting in the kitchen of this twelve room facility and can't walk five feet to answer a call button! I am reporting them to the state. Please don't put your loved one's here. I am so disappointed because my mom really thought the owner cared about his people and now we find he does not!

I worked for this facility in 2006. I was a CNA. This facility is clean, caring and fun. Great people, awesome atmosphere and more. I could continue on but it's all summed up that if I had to,my loved ones would be placed here.

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
