    Unbelievable! My 89 year old mother with advanced dementia was not given proper surveillance for her condition. I got 5 phone calls in 9 days that she had fallen. When relaying how the situation was avoided when in my care, upon a visit I find it had not been followed. Along with a few other minor issues. To top it off when she had a doctors appointment they set up for transportation, it was emphasized several times I would be at the appointment and would meet them. I get there and do not see her, I go to the Dr's office, they are extremely busy so I look around...no mom. This get repeated while I place calls to Wesley. One call "she left 5 minutes ago" (We are 20 min late) second person says she left about 40 minutes ago, (we are only 15min from facility). I assume traffic and call up to Dr and ask if we will ba able to still make appointment, Ah thank goodness they find her back in a room. With the dementia she has no idea what state she is in let alone answer questions. Dr. informs me that the cabulance had NO Instructs on her meeting someone. She was left and during all this time I had no one from Wesley calling to see what happen to her. I verified with the cabulance company Wesley gave them nothing. After seeing mom safely in cabulance and getting a call that she made by the driver and a front desk person that she made it, I call Wesleys Social worker. After my frantic worry and describing my frustration I am informed that mom has to leave TODAY. (We were told 5 days from this and of all the changes she would need to be safe). I was told by this so called "Social worker" that to accommodate mom that was impossible. She actually LIED and said her insurance was out. She demanded I come in immediately and sign paperwork. Not being stupid I contact others at the facility who were incredible (Not all are bad). This so called "social worker" lied and said I requested mom be discharged...hmm shouldn't lie when you sent in writing. I had even stated that we were not ready and did not have the safety features she needs and that they were not being delivered for a day or two.....she showed no emotion and did not care one bit.
