I'd give this place zero stars if I could. My mother and I visited this facility for a tour because my grandparents are requiring more care than the assisted living facility they currently live in can give. The normal person who gives tours was busy so the secretary took us around, handed us off to someone who voiced to us that she did not want to be giving us the tour, then we were handed back to the secretary. Neither of these people knew anything about the facility and could not answer any of our questions. It was a complete waste of time, the rooms are identical to hospital rooms and they cannot have their own furniture, and the environment was extremely unfriendly. My grandparents will not go within 100 yards of this facility.

My mom was there after hip surgery and her care was marginal at best. At times they "forgot" to take her to meals or bring her meals. When helping her move they would hurt her and one aide twisted her hip and caused her a lot of pain. I felt that they did not keep the facility clean and it looked run down. Go somewhere else.

Always short staffed, call lights stay on forever, and incredibly rude staff and management. DO NOT take your loved ones here!!

The place is a dump. Always short staffed, lots of turnover, especially management. Very poorly managed, especially the new dimwitted administrator Ken. Soundcare likes to play hide the pea from the State. Recently had 26 citations from the state, F's, G's, and IJ's

Very concern with the lack of staffing at UP. something has changed over the last few months. The food has gone down hill, lights for help stay on for unreasonable amount of time. Lack of leadership. Lack of care for the resident is below average. This center was recommended very highly to only be a disappointment. I hope they can address issues and get back to being a good care center..

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
