Worst place to take your loved ones! The Care givers don't care about the residents. I went in there to visit and pressed the call button because the resident needed to use the restroom and they took more than an hour to actually come to the room and check what he needed! Thank God he is not there anymore he is so much happier at his new place.

My mother is currently in a this facility,I would like to extend a personal thank you to a gentleman named Steve for taking care and comforting her on her road to recovery! And to the kitchen put some effort into making the food warming enough to eat! "Steve" thank you sir!!!

I had the privilege of staying for a little over a month in the Landmark facility in Yakima, WA. I have nothing but good things to say about the care that I received there. My nursing staff was second to none and my re-hab therapists, Darren and Chanin were the "BEST"! They pushed me even when I told them that I couldn't do it, but pushed in a gentle and loving way. THANK YOU! My only criticism would be the food. The quality was not what I would have expected from a facility as high in quality as Landmark is. I will recommend Landmark to anyone I know who requires either nursing or re-hab care. THANK YOU again for making a terrible recovery go as smooth as possible. Glenda Nierman-McKenzie

Fines, Complaints, and Inspection Problems in the Past 3 Years

Compare The Number of Problems

Types of Problems at Nursing Homes

Some issues within a nursing home are much more severe than others. Medicare evaluates each problem based on 2 scales: the number of residents affected by a problem and the severity of the potential or actual harm to residents based on the problem. We have color coded the matrix below to make it easier to pick out the more severe problems. In general, orange and red issues related to the treatment of a resident are considered substandard quality of care.

  Residents Affected
Severity of the Deficiency Few Some Many
Immediate jeopardy to resident health or safety J K L
Actual harm that is not immediate jeopardy G H I
No actual harm with potential for more than minimal harm that is not immediate jeopardy D E F
No actual harm with potential for minimal harm A B C
