    We just recently placed our 89 year-old mom at Heritage Square for Rehab and were horrified by the operation of this facility!!! Her doctor ordered that she receive assistance walking several times per day to regain her strength, but instead she was left lying in her bed all day and night. She was told by the staff that she was not able to use the restroom without assistance. When she requested assistance, each time it took two or more hours before anybody responded. On one occasion she was unable to wait and it resulted in her bedding and nightgown being soaked in urine. She repeatedly rang for assistance and finally after lying in the wet mess for a couple hours two staff members came to her room. Instead of moving her into a wheelchair, they left her in the bed and grabbed her arms and legs to move her from side to side while they removed and replaced the sheets. Mom suffers from edema and severe swelling in her legs and was screaming in excruciating pain the entire time. They left her in the soiled nightgown when they were finished. During the night shift there were only three males on staff, no female workers, which we felt was highly inappropriate for elderly female patients. Please heed our warning and do not allow your loved ones to suffer at this facility the way our mom did!!! Jody C. 1/27/16

    For being a place for rehab, this is an awful facility and I recommend placing a loved one anywhere but here. My grandma was here for rehab to get stronger, sat in a wheel chair all day long, when rang to use the bathroom had too wait longer than 30 min. Many times. And when someone did come they would proceed to push the wheelchair to the bathroom when written ALL OVER her room, ' per therapy walk to bathroom every time' several staff said I didn't know that, then gave attitude when family asked them to walk her. Hello! You work in a rehab facility to help people get stronger, not make them weaker!... My grandma was there 1 month and left weak just like when she arrived.. Sad excuse for a rehab facility.

    My mom was there for about 1 week three years ago and my dad was there twice after he had knee replacement surgery. Both my parents had a great experience at Golden Living Center in Greendale. Thank You!

    Horrible place. Staff does not know what the are doing. My mom's doctor pulled her out of there because she ended up with a staph infection and pneumonia.

    Terrible Place. My Mother asked REPEATEDLY for a Bottle of Water, Still hadn't received any 3 hours later. I called and the Lady that answered the Phone said "I'll take care of that right away" , Never even asked My Mother's Name OR What her Room # was - Just TRIED to "blow me off " the Phone. My Mother rang for Bathroom help NUMEROUS TIMES, No one ever came - Finally defecated all over herself, Hours later when they saw it they told her to just lie still and not touch anything. They never came back - Just left her lying there in it!!! Medications mixed up also - She asked for something to eat, Had to ask again 3 hours later and was told "Well You told us you didn't want anything "!! The 2nd day she was in there I told her to pack up and I would be picking her up. I called AHEAD and told them that I would be taking her out of there, When I tried to leave they chased me all the way out to my Car telling me I had to sign some paperwork!!! 3 Days later and NEVER EVEN received a Call from the Director to ask what had happened OR WHY My Mother left, No apology, No concern WHATSOEVER!!!! I am going to file a complaint with the appropriate DQA Government Agency/Medical Board. If they had ZERO STARTS I would have selected that as they do not even warrant ONE STAR!!! They SHOULD BE SHUT DOWN!!!!! HORRIBLE PLACE!!!
